Student immunizations require communication and follow up by Marion
County School Board, Marion County Health Department, schools and parents.
Each student must have a “Certificate of Immunization” (DH 680) on file.
The school district may issue a 30-day waiver form permitting the parent
to obtain the “Certificate of Immunization” form. The school nurse assigned
to the school will be available to serve as a resource to both the school
